To make sure your plan is one that we accept, you can call the number on the back of your insurance card or contact our billing department (352) 289-2167.

To refill your prescription you can:
Have your pharmacy send a E-refill request (normally ready within 1-2 hours)
Send a request through your Helow Portal (normally ready within 2-4 hours)
Call and speak with one our our nurses (normally ready within 1-3 days)
Have your pharmacy send a fax request (normally ready within 1-3 days)
To maintain your prescription you must be seen every 6 months regarding your medication. If your prescription is considered a controlled medication, you must be seen every 3 months.

To ensure quality health care, we require all patients have labs and and an annual physical to remain compliant with the practice.

Wake up and feel sick? Walk in!
Our walk-in clinic provides same day treatment for a single acute illness or injury and typically seen within an hour of arriving, most are seen within 10 minutes if they arrive in the afternoon.
The hours are Monday-Friday from 8am-4:45pm and Saturdays from 8am-11:45am. We also offer access to a nurse on-call 24/7.
Walk-in’s are only available for established, compliant patients.
We cannot provide:
Medication refills
Test results
Treatment for chronic conditions

To qualify for a parking permit, patients must meet specific criteria and schedule an appointment to discuss obtaining a pass:
Use of an assistive device such as a cane, walker, crutches, or wheelchair, which must be in use at the appointment for documentation.
Use of supplemental oxygen around the clock.
Class 3 or 4 heart failure.
Neurologic conditions such as Parkinson’s or multiple sclerosis.
Temporary permits (up to 90 days) may be available for patients undergoing orthopedic surgery, other surgeries, or procedures.
Conditions that do not qualify: obesity, back pain, joint pain, and fatigue.

If you leave a voicemail with the clinical staff, you can expect a call by by the end of business day. Leaving multiple voicemails may cause a delay in response time. If you have not been contacted by the following morning, please ask to speak to a manager.

If your surgeon has requested that you have to be cleared by your primary care provider, then YES.
